Department of Defense Pharmacy Uniform Formulary Blanket Purchase Agreement/Uniform Formulary Additional Discount Program

The Department of Defense (DoD) is seeking quotations for pharmaceutical agents to be included in the DoD Uniform Formulary (UF). The UF is a list of medications that are available to DoD beneficiaries. The DoD Pharmacy and Therapeutics (P&T) Committee will review the quotations and make recommendations for the inclusion of pharmaceutical agents on the UF. The DoD is looking for companies to provide refunds for pharmaceutical agents dispensed to DoD beneficiaries through the TRICARE retail network. • The DoD P&T Committee will consider the relative clinical effectiveness and relative cost effectiveness of pharmaceutical agents in recommending the selection of agents for the UF. • The DoD P&T Committee will consider the refund quotes for UF ADPs as part of its evaluation of the relative cost effectiveness of pharmaceutical agents. • The agreement will be effective upon signature of the Contracting Officer and will continue until the effective date of any change in the classification of the pharmaceutical agent(s) or is otherwise terminated. • The DoD reserves the right to reject or decline to accept specific formulations or NDC-11s of a given pharmaceutical agent included on any UF BPA and/or UF ADP quote. • The DoD will not consider UF BPA or UF ADP quotes that are contingent upon market shares.
