Description
Rockland BOCES seeks a qualified provider to deliver on‑site physical therapy services to students across multiple school sites during the 2026‑2027 school year. The contractor must evaluate, treat, and document services in compliance with Individualized Education Programs (IEPs) and Medicaid requirements, travel between locations, and maintain rigorous insurance and certification standards.
• Provide direct evaluation and treatment for school‑aged children with disabilities during regular school hours.
• Maintain same‑day documentation in the Kinney system and quarterly IEP updates.
• Submit a detailed fee schedule (30‑, 45‑, 60‑minute rates, evaluation, meeting, mileage) and meet extensive insurance, background, and training certifications.
