DHS / Outreach & Engagement (TMaH)

The Minnesota Department of Human Services is seeking proposals from a qualified contractor to conduct community outreach and engagement to support improving maternal health whole-person care delivery, quality improvement and safety, and access infrastructure and workforce in Hennepin County in alignment with the Transforming Maternal Health (TMaH) model. The selected contractor will facilitate a short-term Medicaid perinatal beneficiary advisory council and produce a recommendations report for the Minnesota Department of Human Services, TMaH leadership, and maternal health partners. Some key points of this RFP include: • The contractor will facilitate a short-term Medicaid perinatal beneficiary advisory council (PBAC) and produce a recommendations report. • The PBAC must be comprised of current/former Medicaid beneficiaries, family members, and caregivers. • The report will summarize identified strategies to improve navigation and experience of Medicaid & CHIP services for perinatal populations, access to perinatal benefits, delivery of whole-person care, and reduction of coverage gaps. • The contractor will provide technical assistance to TMaH providers and partners on improving access, patient experience, and shared decision-making. • The contractor will present at meetings, trainings, or conferences and provide other support as needed.
