Department of Defense Pharmacy Uniform Formulary Blanket Purchase Agreement / Uniform Formulary Additional Discount Program

The Department of Defense (DoD) is seeking quotes for pharmaceutical agents to be included in the Uniform Formulary (UF) and the Basic Core Formulary (BCF) or Extended Core Formulary (ECF). The DoD Pharmacy and Therapeutics (P&T) Committee will consider the relative clinical effectiveness and relative cost effectiveness of pharmaceutical agents in recommending the selection of agents for the UF and the classification of a pharmaceutical agent in the generic, formulary, or non-formulary cost share tier. The quotes should include prices for the Military Treatment Facilities (MTFs) and the TRICARE Mail Order Pharmacy (TMOP), as well as refunds for the TRICARE retail network. Key points to consider: • The DoD P&T Committee will review the quotes and make recommendations for the UF and BCF/ECF. • The quotes should include prices for all National Drug Codes (NDCs) available for purchase by the Government. • The Company agrees to provide refunds to the Government based on the accrued utilization of the pharmaceutical agent(s) listed in the UF ADP Appendix. • The agreement will continue until the effective date of any change in the classification of the pharmaceutical agent(s) contained in the agreement, or is otherwise terminated.
