Vermont Crisis Standards of Care Plan Update RFP

The Vermont Department of Health is seeking to establish contracts with one or more companies to update the Vermont Crisis Standards of Care Plan. The selected bidder(s) will be expected to sign a contract with the State, including the Standard Contract Form and Attachment C. The contract will obligate the bidder to provide the services and/or products identified in its bid, at the prices listed. The project requirements and deliverables are outlined in Attachment A of the sample contract. • The contract period is expected to be six months, with a start date of May 11, 2026. • The State has budgeted up to $35,000 for this project. • The bidder must provide a Cover Letter, Technical Response, and Price Schedule as part of their bid. • The bidder must also provide a Certificate of Compliance, Form W-9, and Certificate of Insurance. • The State reserves the right to evaluate responses and select the bidder(s) as may be determined to be in the best interest of the State.
