Q301--Prostate Testing for Salt Lake City VA Medical Center

The Department of Veterans Affairs, Veterans Health Administration, Salt Lake City VA Medical Center has a requirement for prostate testing and reporting services. The contractor will provide prostate testing, analysis, and result interpretation, as well as provide individual patient-chartable test result reports meeting Clinical Laboratory Improvement Amendments (CLIA) requirements. The contract will be awarded as a Firm-Fixed Price, 5-Year Blanket Purchase Agreement with one year ordering periods. Key aspects of the job include: • Providing laboratory testing, analysis, and result interpretation for prostate testing • Generating a dynamic report with patient information, specimen information, and genomic score interpretation • Providing test result reports meeting CLIA requirements • Complying with all local, state, and federal laws and regulations • Maintaining safety and health standards consistent with OSHA and CDC requirements • Providing a copy of all relevant permits, licenses, and certifications prior to contract award
