• Provide CPT‑coded hearing‑aid selection (92631), fitting (92634), and post‑fitting follow‑up (92636) services at capped rates.
• Supply ear molds, hearing‑aid devices (up to $500 per unit), and repair services (up to $240 per device) with a minimum one‑year warranty.
