HPSA Zip Code Crosswalk

The Centers for Medicare and Medicaid Services (CMS) is seeking a contractor to develop and provide a crosswalk of geographic Health Professional Shortage Area (HPSA) designations to United States Postal Service (USPS) ZIP Codes. The purpose of this contract is to determine the payment of the Medicare HPSA bonus. The contractor will be required to furnish the necessary services, personnel, materials, equipment, and facilities to perform this contract. The contract will have a base period and four option periods, with a total period of performance of 5 years.
