Rehabilitation Therapy Services at Badger Prairie Health Care Center

The County of Dane is seeking a rehabilitation agency to provide physical therapy, occupational therapy, and speech-language therapy services to residents at the Badger Prairie Health Care Center. The selected agency will be responsible for delivering high-quality, person-centered therapy that meets the clinical needs of residents in accordance with facility care plans and regulatory requirements. The agency must have a minimum of 3-5 years of experience providing rehabilitation therapy services to long-term care facilities, particularly those specializing in geriatrics and residents with severe chronic mental illness. Key responsibilities include providing timely initial assessments, ongoing screening, evaluations, and treatment programs, as well as ensuring that all therapy services are reimbursable by the appropriate payer sources. * Provide physical therapy, occupational therapy, and speech-language therapy services to residents * Deliver high-quality, person-centered therapy that meets clinical needs and regulatory requirements * Assign professional staff and certified assistants to the facility * Ensure timely initial assessments, ongoing screening, evaluations, and treatment programs * Provide pre-approved non-billable services at facility expense * Assist the facility in obtaining necessary prior authorizations
