Life Insurance and AD&D Coverage

The City of Hialeah is seeking proposals from qualified firms to provide basic group term life and supplemental term life insurance with accidental death and dismemberment coverage for a period of 36 months. The City has approximately 1500 employees and is looking for a comprehensive approach to provide life insurance and AD&D coverage. The selected respondent will be required to provide proof of experience, qualifications, and financial strength. The proposal should include a description of the systematic approach and actions to be taken to provide the services, as well as a pricing proposal that includes all commissions, fees, and administrative costs. • The contract term is for 36 months. • The City has approximately 1500 employees. • The selected respondent must provide proof of experience and qualifications. • The proposal should include a pricing proposal that includes all commissions, fees, and administrative costs. • The City reserves the right to reject any and all responses, and to decide to re-advertise/re-bid or not, or to waive any informalities or minor irregularities, technicalities, or omissions, in the solicitation process.
