Residential Behavioral Health Services - Medium Acuity

The Dane County Department of Human Services is seeking proposals from qualified providers to operate a community-based residential facility for adults with behavioral health needs. The facility will provide medium-acuity level of care, focusing on stabilization, skill-building, and therapeutic interventions to support movement toward greater independence. The provider will be responsible for assessing residents' needs, developing individualized service plans, and providing 24/7 care and supervision. The contract term is one year, with an option to renew for four additional one-year periods. Some key requirements for the provider include: • Providing a supportive structure that follows the "teach to independence" coaching model • Offering daily and weekly programming that supports residents' ability to practice skills needed for independent living • Collaborating with the resident's treatment team and other behavioral health providers • Maintaining licensure under Wisconsin Administrative Code DHS 83 for CBRFs or DHS 88 for 3-4 Bed AFH • Complying with all applicable laws, regulations, and standards, including those related to data privacy and security.
