Description
Santa Fe County’s Community Services Department is seeking a New Mexico‑licensed behavioral‑health provider to deliver comprehensive, community‑based support services for youth (ages 12‑18) and their families who are participants in the County’s Teen Court program. Services include assessments using the CONNECT SDOH tool, individualized and group counseling, case management, school‑based interventions, intensive home‑based support, coordination with Teen Court, and data sharing via the Unite Us platform. The contractor must comply with HIPAA, County living‑wage ordinances, and provide quarterly reporting. The contract is a one‑year base period with up to three one‑year renewal options (maximum four years).
