The State of Idaho’s Transportation Department is seeking a qualified printer to produce and deliver specialized DMV forms on an as‑needed basis. The contract covers four distinct secure forms that require specialty security paper, multi‑color Pantone inks, watermarks, and erasure‑sensitive features. Minimum order quantities are set per case and deliveries must be made within 30 days of order receipt, with the contract initially spanning one year and renewable up to a total of five years, capped at $500,000.
• Produce Form 3582 (secure Power of Attorney), Form 3584 (secure Reassignment/Disclosure), and Form 0502 (Application for Report of Sale & Certificate of Title) on specified security paper and inks.
• Meet minimum batch sizes (192 cases of Form 3582, 48 cases of Form 3584, 192 cases of Form 0502) and ship palletized, stretch‑wrapped to the Central Supply Warehouse in Boise, Idaho.
• Provide required insurance certificates within five business days of award and maintain coverage for the contract’s life; comply with all Idaho procurement, trade‑secret, and data‑security requirements.
