Description

The Department of Veterans Affairs seeks a qualified contractor to operate a Community‑Based Outpatient Clinic (CBOC) in Sherman, Texas, delivering comprehensive primary‑care services to Veterans under a per‑member‑per‑month (PMPM) reimbursement model. The contract is a fixed‑price IDIQ with a base year and up to nine optional one‑year extensions, allowing the government to issue task orders for up to 12 months each. The contractor must furnish clinic space, staffing, equipment (including a fixed‑room digital radiography system), medication/vaccine inventories, and all ancillary services such as point‑of‑care testing, pharmacy fulfillment, and tele‑health support. Performance will be measured against VA‑specified quality and access metrics, and the contractor must meet strict staffing ratios, credentialing, and regulatory compliance requirements. • Provide primary‑care, women’s‑health, and mental‑health integration services on a PMPM basis for an estimated veteran roster of ~4,300–6,700 patients. • Supply and maintain on‑site plain‑film X‑ray equipment, medication/vaccine stocks, clinic furnishings, IT/tele‑health infrastructure, and backup staffing. • Meet VA performance targets (e.g., ≥ 94 % established‑patient appointments within 20 days, ≥ 78 % cardiovascular/diabetes risk‑management, ≥ 75 % veteran satisfaction) and adhere to all FAR, VA, and regulatory clauses.

Budget estimate

$581,175 – $70,000,000

The contract ceiling is $70 million with a guaranteed minimum of $581,175. PMPM rates range from $4,305 in the base year up to $6,678 in the ninth option year. Assuming an average roster of ~5,000 Veterans, annual revenues could range from roughly $260 million (if all options are exercised and maximum rates applied) down to the minimum $581,175 guaranteed spend. Realistically, annual spend will fall between the low‑end guaranteed amount and the mid‑range of the ceiling, likely $5–15 million per year for a contractor that secures most task orders.
