Description
The Department of Health, Hawaii State Hospital, is seeking a contractor to provide pest control services for the hospital's campus. The contractor will be required to attend a mandatory pre-conference and site visit to evaluate the scope of work. The contract will be for a period of 12 months, with the option for renewal. The contractor must be compliant with all applicable state and federal statutes, and provide proof of compliance through Hawaii Compliance Express (HCE) or paper documentation.
• The contractor must provide monthly pest control services for the hospital's buildings and grounds.
• The contractor must be registered in HCE and have a valid certificate of good standing.
• The contractor must include all applicable taxes, fees, and charges in the price submitted.
• The contractor must comply with all state and federal statutes, including the Hawaii General Excise Tax (GET) and worker's compensation laws.
• The contractor must provide at least three references for similar services performed in the State of Hawaii within the past 5 years.
