Projects for Assistance in Transition From Homelessness (PATH)

The Capital Area Human Services District (CAHSD) is seeking proposals for the Projects for Assistance in Transition From Homelessness (PATH) program. The program aims to provide outreach services, case management, and assistance in obtaining and coordinating social and maintenance services for eligible individuals who experience homelessness. The successful proposer will be required to provide services to adults with serious mental illnesses who are homeless or at risk of becoming homeless. The contract will be for a period of up to 12 months, with the option to extend for two additional 12-month periods. The proposer must have experience in working with similar programs and must be able to demonstrate their ability to provide the required services. Some key features of the job and scope-of-work include: • Outreach services to those with serious mental illness who are experiencing homelessness or in danger of becoming homeless • Case management services including preparing and implementing a plan for the provision of community mental health services to eligible homeless individuals • Assistance in obtaining and coordinating social and maintenance services for eligible individuals who experience homelessness • Assistance to eligible individuals who experience homelessness in obtaining income support services, including housing assistance, food stamps, and supplemental security income benefits • Referral services to eligible individuals who experience homelessness for such other services as may be appropriate
