• 300 printed pamphlets (5½ × 7¾ in, 52 pages, 4‑color process on 8‑pt white product‑olith).
• Full finishing: acrylic OPP+ lamination on both sides, die‑cut stair‑stepped tabs (~50), black durable coil binding, shrink‑film packaging (15 units per pack).
• Required pre‑production sample (one finished copy) shipped to Dr. Lashaun Smith, Bronx, NY; proof approval turnaround ≤ 5 workdays.
• Mandatory submission of a W‑9; invoice due within 30 days of delivery; electronic PDF file will be provided by the Government.
