Description
The Colorado Department of Health Care Policy and Financing is seeking a contractor to provide Housing Navigation Services for Region 2B, which includes Crowley, Kiowa, Otero, Bent, Prowers, and Baca counties. The contractor will be responsible for coordinating rental assistance vouchers, administering housing navigation services, and providing community outreach. The contractor will work with Medicaid members, transition coordinators, and landlords to identify and secure affordable and accessible housing options. The contract term is anticipated to be one year, with a start-up period beginning on January 1, 2026, and ending on February 1, 2026. The initial operational period is expected to begin at the end of the start-up period and end on December 31, 2026. Key responsibilities include:
• Coordinating rental assistance vouchers and submitting them to the Department of Housing
• Administering housing navigation services, including building relationships with the housing community and providing information to community entities
• Providing community outreach, including identifying and developing affordable and accessible housing options
• Submitting monthly reports, including a Monthly Housing Navigation Services Report and a Monthly Community Outreach Report
• Participating in evaluation and monitoring activities to ensure compliance with the contract terms.
