The Department of Veterans Affairs, Network Contracting Office (NCO) 21, is conducting market research to identify potential vendors that can provide Health Care for Homeless Veterans (HCHV) Contracted Emergency Residential Services (CERS) for Homeless Geriatric Veterans in San Mateo, Santa Clara, or Southern Alameda County. The contractor shall primarily focus on providing emergency housing and supportive services for homeless geriatric Veterans in accordance with the HCHV CERS model requirements. The contractor should have a facility located in San Mateo, Santa Clara, or Southern Alameda County. Key services to be provided include: • Residential room and board • Dietetic services • Laundry facilities • Case management services • Medical case management services • Permanent housing search • Financial planning and employment • Group activities • Discharge planning • Medication management The period of performance will be for 5 years, and the contractor will be expected to provide all labor, supervision, housing, material, and supplies necessary to provide these services.
