Aging and Disability Services

Ramsey County’s Aging & Disability Services Division seeks qualified agencies to deliver comprehensive case management services to county residents participating in a suite of waiver and support programs. The work emphasizes voluntary, person‑centered practices across multiple service lines, including alternative care, brain injury, developmental disability, elderly, and vulnerable adult programs. Successful contractors will provide individualized case planning, coordination of community resources, monitoring, and outcome reporting. • Provide case management for Alternative Care, Brain Injury Waiver, Community Access for Disability Inclusion Waiver, Community Alternative Care Waiver, Developmental Disability Waiver, Elderly Waiver, Essential Community Supports Rule 185, and Vulnerable Adults/Developmental Disability Targeted services. • Deliver services using person‑centered, voluntary practices, ensuring compliance with Ramsey County policies and state waiver regulations. • Maintain documentation, data reporting, and performance metrics for each program, supporting ongoing oversight and quality improvement.
