Description
The Defense Health Agency (DHA) is seeking a qualified contractor to establish a five‑year Blanket Purchase Agreement (BPA) to provide comprehensive medical laboratory testing services for the 18th Medical Group at Kadena Air Base, Japan. The contractor must supply all management, tools, supplies, equipment, and labor required to perform a defined set of reference laboratory tests (fungal cultures, TB testing, antifungal susceptibility, etc.) at the base. The BPA will cover the period of performance from 01 Dec 2026 through 30 Nov 2031, with individual line items each quoted on a “each” basis. Offers are to be submitted electronically by 10:00 am Pacific on 26 Oct 2026; questions are due by 28 Sep 2026. The award will be made to the most advantageous (best‑value) offer, with past performance weighted more heavily than price.
• Scope: Perform and deliver specified medical laboratory tests (fungal culture, TB, antifungal susceptibility, etc.) for U.S. military personnel at Kadena AB.
• Delivery: Services performed on‑site; contractor provides all necessary equipment and supplies.
• Contract type: Firm‑fixed‑price BPA, 5‑year term, multiple CLINs each “per each” unit.
• Evaluation: Technical capability (accept/reject), past performance (primary factor), price (secondary).
• Submission: Electronic PDF quote to April L. Delmendo; questions via email by 28 Sep 2026.
• Mandatory compliance: DBIDS pass, SAM registration, WAWF invoicing, adherence to numerous FAR/DFARS clauses.
