Description
The Colorado Department of Health Care Policy and Financing is seeking a contractor to provide Housing Navigation Services for Region 2B, which includes Crowley, Kiowa, Otero, Bent, Prowers, and Baca counties. The contractor will be responsible for coordinating rental assistance vouchers, administering housing navigation services, and providing community outreach. The contractor will also be required to submit monthly reports and participate in evaluation meetings. The goal of the project is to ensure improved housing search and placement for individuals receiving Targeted Case Management-Transition Coordination. Some key responsibilities include:
• Coordinating rental assistance vouchers and submitting them to the Colorado Division of Housing
• Building relationships with the housing community to increase awareness of limited housing options
• Providing information to community entities who may be sources of housing opportunities
• Networking with landlords, housing agencies, and community organizations to promote identification and development of affordable and accessible housing
• Identifying newly constructed or developed units that have wheelchair and/or hearing/visual impairment accessible units
• Submitting monthly reports, including a monthly housing navigation services report and a monthly community outreach report
• Participating in evaluation meetings and providing feedback on the housing navigation services
