6505--Budesonide/Formoterol INH

The Department of Veterans Affairs is seeking a supplier for Budesonide/Formoterol inhalers. The contract will be for a base year with four one-year option periods. The supplier must provide the products at the awarded prices for the facilities/agencies listed, including all Department of Veterans Affairs facilities, Department of Defense facilities, Indian Health Service facilities, and Bureau of Prisons facilities. The offeror must submit a price for the base year and all four option years for line items 1 and 2.
