Community-based Human Services for Older Adults and Individuals with Disabilities

This contract provides community-based human services for older adults and individuals with disabilities in Bergen County, New Jersey. The services include certified home health aide programs, assisted living programs, care management, and respite care. The contract is valued at $558,196.00 and is set to expire on December 31, 2025. The contract has the option to be renewed for an additional term. The services will be provided by various vendors, including Geriatric Services, Inc., Heightened Independence & Progress (HIP), The Arc of Bergen & Passaic Counties, Inc., and Spectrum for Living Group Homes, Inc. • Certified Home Health Aide Program: provides certified home health aide hours to older individuals and disabled adults who are at risk of nursing home placement. • Assisted Living Program: provides assisted living services to individuals who are fully or partially dependent in several Activities of Daily Living (ADLs) and who are deemed qualified for nursing home care. • Care Management: provides care management, including the development and implementation of a care plan for arranging and coordinating the delivery of services. • Respite Care: provides short-term or intermittent services provided for adults or children with disabilities either in their own homes or in other facilities, in order to provide relief to those persons normally providing care.
