DATA COLLECTION SURVEY FOR SC BRFSS

This contract is for the collection of telephone‑survey data for the South Carolina Behavioral Risk Factor Surveillance System (SC BRFSS). The awardee will be responsible for executing the CDC‑approved BRFSS protocol, including sample management, interviewer training, calling schedules, disposition coding, quality‑monitoring, and secure data submission to the state health department. The work is performed under strict confidentiality and OMB burden‑estimate guidelines, and it must adhere to the detailed calling‑attempt and callback rules defined by CDC. • Scope of work: telephone interview administration (landline & cell‑phone), eligibility screening, random‑adult selection, and recording of health‑behavior responses. • Required deliverables: monthly sample files, quarterly data‑quality reports, and final cleaned dataset submitted by the quarter‑end deadlines. • Compliance obligations: follow CDC’s disposition‑code system, maintain USGCB security standards, and destroy sample files after data collection is complete.
