The Multnomah County Health Department is seeking organizations to provide services for HIV/AIDS patients, including ambulatory/medical care, health insurance, mental health treatment and peer support, oral health/dental care, medical case management, and other services. The contract is funded by the Ryan White Part A federal grant and program income generated from the Oregon Health Authority's Ryan White Part B grant. The services must be coordinated with other Ryan White-funded programs and must be accessible to cultural and linguistic minorities, sexual minorities, gender minorities, women, youth, and children. The proposed program must include an outreach plan and a plan to reach and engage clients who are out of care or have a difficult time accessing services. The contract will be awarded to multiple vendors, and the selection criteria include the proposer's experience and qualifications, the proposed program's consistency with program expectations, and the proposed model of service delivery.
• The contract is for a period of five years, with an estimated annual funding of $6.4 million.
• The services must be provided to residents living in the Portland Transitional Grant Area (TGA), which includes Clackamas, Columbia, Multnomah, Washington, and Yamhill counties in Oregon, and Clark County in Washington.
• The proposer must demonstrate their capacity to provide the proposed services, including the number of Ryan White and leveraged staff needed to support the effort and the estimated number of clients served within a given contract year.
• The proposer must also demonstrate their ability to coordinate services with other HIV service providers in the Portland TGA and to communicate with other agencies providing HIV care services to avoid duplication and assure coordination of services.
• The contract includes performance measures and program outcomes, including viral suppression rates and lab visits, and the proposer must demonstrate their ability to meet these measures and outcomes.
