The City of Goodyear has entered into a contract with FMLA Source, Inc. for the administration of Family and Medical Leave Act (FMLA) services. The contract includes the provision of account management, service access, and FMLA administration services. The contract has a term of one year, with the option to renew for additional successive 12-month terms. The contract also includes provisions for termination, fees, and confidentiality. The key services provided by FMLA Source, Inc. include: • Account Management: assignment of an account manager to serve as the contact person and provide reports and feedback on the services • Service Access: toll-free access to FMLA experts • FMLA Administration: administration of FMLA eligibility, leave requests, and conditional approvals, final approvals, and denial notifications • Additional Leave Types: tracking of employee absences/leaves resulting from jury duty and military leave (non-FMLA) The contract also includes provisions for confidentiality, non-discrimination, and compliance with applicable laws and regulations.
