This contract awards a fixed‑price, five‑year (with up to four one‑year renewal options) statewide rehabilitation services program for individuals with traumatic brain injury (TBI) and traumatic spinal cord injury (SCI). The contractor must be CARF‑accredited for both inpatient and outpatient brain‑injury and spinal‑cord‑injury programs, provide all required HIPAA Business Associate Agreements, and submit detailed invoices and discharge summaries within ten business days of patient discharge. Funding is allocated per‑service with maximum caps (e.g., $34,400 maximum for inpatient TBI, $77,650 for inpatient SCI, and defined outpatient rates). The State requires the contractor to maintain extensive insurance coverage, adhere to strict reporting and record‑retention rules, and cannot use subcontractors.
