This contract funds a transportation component (“Autos for Access”) of a women’s residential 12‑Step recovery program administered by The Gatehouse Hammond in Lake County, Indiana. The grant supplies up to $25,000 to cover vehicle purchase, insurance, fuel, maintenance, driver labor, and related costs so that women 18 + in recovery can reliably travel to medical, psychiatric, pharmacy, grocery, and 12‑Step appointments. The agreement runs until December 31 2026 and requires monthly performance reports detailing numbers of transports, nutrition instruction sessions, chronic‑disease screenings, and 12‑Step meeting attendance. Payments are split into two $12,500 installments, with the first due within 45 days of signing (after serving the first 20 attendees) and the second due four months later. The contract can be terminated for cause or with 90‑day notice without cause, and any unspent funds must be returned unless rolled into a renewed agreement.
