Description
The Department of Veterans Affairs is seeking quotes for the provision of Ostomy and Gastrostomy supplies to the Veterans Healthcare Administration Salt Lake City Health Care System. The contract will be for a base year with four one-year options. The contractor will be required to provide pricing for the supplies listed in the attachment. The Government reserves the right to make an award based on the initial quotes received.
• The contract is for an indefinite quantity with a guaranteed minimum of $25,000 and a maximum of $4,000,000 over the life of the contract.
• The contractor must be an original equipment manufacturer, authorized dealer, authorized distributor, or authorized reseller for the proposed medical supplies.
• The contractor must provide a copy of their company's information security policies and training for staff.
• The contractor must complete and sign the VAAR Clause 852.219-76, VA Notice of LOS-Certificate of Compliance for Supplies and Products, and the RFO 52.225-2, Buy American Certificate.
• The contractor must provide at least three recent and relevant past performance references.
