Transitional Housing Placement Plus (THP+)

The County of San Mateo's Human Services Agency (HSA) is seeking proposals from organizations to deliver Transitional Housing Program-Plus (THP-Plus) services to former foster or probation youth aged 18-25. The goal of this program is to prevent homelessness, provide age-appropriate housing, support education and employment, and foster self-sufficiency and long-term stability. The selected contractor will implement the program as outlined in the RFP, with an initial term of three years and an option to extend up to two additional years. The program includes housing options like Single Site, Host Family, and Scattered Site models, and each participant will work with support staff to develop a Transitional Youth Case Plan (TYCP) focused on education, employment, and life skills development. Key aspects of the program include: • Providing supervised housing and supportive services for up to 36 months • Assisting participants in developing a TYCP to achieve independence • Offering various housing models, including Single Site, Host Family, and Scattered Site • Providing case management, crisis intervention, and therapy referrals • Assisting participants in securing housing and providing subsidies for rent and living expenses • Fostering self-sufficiency and long-term stability among participants
