Respite and Extended Services

The Connecticut Department of Public Health is seeking proposals for a statewide program to provide information and assistance with accessing and funding services for children and youth with special health care needs. The program aims to supplement existing services provided by the Department's CYSHCN program and the CT Medical Home Initiative. The selected organization will be responsible for developing, administering, and serving as the fiduciary of funds for the program. The contract will be for three years with two one-year extensions, beginning July 1, 2026, through June 30, 2031, subject to the availability of funds and satisfactory program performance. Key activities include assisting caregivers in accessing and funding respite services and extended services, supporting services, enrolling CYSHCN in the CHMI program, and obtaining health insurance for CYSHCN.
