Description
The Colorado Department of Health Care Policy and Financing is seeking a contractor to provide Housing Navigation Services for Region 5A, which includes Garfield, Eagle, Pitkin, Mesa, Delta, Montrose, and Gunnison counties. The contractor will be responsible for coordinating rental assistance vouchers, administering housing navigation services, and providing community outreach. The contractor will also be required to submit monthly reports and participate in evaluation and monitoring activities. The goal of the project is to ensure improved housing search and placement for individuals receiving Targeted Case Management-Transition Coordination services. Some key responsibilities of the contractor include:
• Coordinating rental assistance vouchers and submitting them to the Department of Housing
• Providing person-centered collaboration with Transition Coordinators, the Division of Housing, and the Department
• Building relationships with the housing community to increase awareness of limited housing options for Members
• Identifying and developing affordable and accessible housing options for Members
• Submitting monthly reports on housing navigation services, including successful placements and no-fault failed transitions
• Participating in evaluation and monitoring activities to ensure the contractor is meeting the requirements of the contract
