RFP No. ACH-900726 Family Resource Centers - Lead Agencies

The County of Alameda is seeking bids from qualified agencies to provide school-linked health and wellness services to families in Oakland, San Leandro, and Hayward. The selected agencies will work with the County's Family Resource Centers (FRC) Initiative to design and provide services that address inequities of access to health services, public benefits, and culturally appropriate parenting resources. The services will include benefits application support, case management, and family resource navigation, as well as additional services such as parent/family support groups, health and wellness workshops, and referrals to partner agencies. The agencies will also be required to participate in County evaluation, data collection, and continuous quality improvement efforts. Some key points to consider: * The County is seeking bids for two award regions: Mid-County FRCs (San Leandro and Hayward) and Oakland Central FRC. * The agencies will be required to work closely with school districts and community partners to provide services that align with school district priorities and County initiatives. * The agencies will be required to provide services in a linguistically and culturally responsive manner, and to engage families, including those experiencing homelessness, immigrant/newcomer families, and fathers/father-figures. * The agencies will be required to participate in quarterly data review meetings with school district and County staff to ensure continuous quality improvement.
