Description
Erie County’s Department for the Aging is seeking qualified vendors to provide (1) case‑management services for adults 60 + and (2) public information and education outreach throughout Erie County. The contract will run from February 1 2027 to March 31 2030 with possible one‑year extensions. Vendors may submit proposals for either option or a combined offering. Core duties include client assessments, care‑plan development, coordination of services, data entry into the Peerplace system, and delivery of community‑wide educational programs, workshops, and outreach materials. Performance will be measured by client caseloads, timeliness of services, satisfaction scores, event counts, and compliance with NYS‑OFA/EISEP regulations. The award pool is $1.8 million, with the County covering 90 % and vendors providing a 10 % match.
