Description
The Defense Health Agency Contracting Activity (DHACA) Healthcare Contracting Division-West (HCD-W) is seeking quotes for the provision of PIV cards, specifically IDEMIA PIV Card Media and FIPSlink V-PIV - HID TAC. The contractor must provide 5000 PIV cards with a pre-programmed PIVFASCN number readable by Monitor Dynamic’s SafeNET security system. The cards must be delivered to Madigan Army Medical Center within 30 days of the award. The contractor must also provide 5 IDEMIA PIV Card Media (Cosmos 8.2 Javacard + ID-One v2.4 PIV Applet) to the same location.
• The contractor must be an original equipment manufacturer (OEM), authorized dealer, authorized distributor, or authorized reseller verified by an authorization letter or other document(s) from the OEM.
• The contractor must be registered in the System for Award Management (SAM) with the appropriate NAICS code and completed representations and certifications.
• The contractor must provide all documents listed and comply with all RFQ instructions.
• The Government intends to award a firm-fixed-price (FFP) contract resulting from this RFQ to the responsible Quoter, whose quote conforms to the solicitation and is the most advantageous (best value) to the Government, price and other factors considered.
