OPEN-ENDED - SCHOOL BASED HEALTH CENTER SERVICES (SBHC) RFP

The New York City Department of Health and Mental Hygiene is seeking qualified providers to deliver comprehensive primary and mental health services through School‑Based Health Centers (SBHC) located in high‑risk NYC schools. This open‑ended RFP allows ongoing submissions until the city’s needs are satisfied, focusing on preventive care, chronic disease management, and acute care for students on campus. Providers must be pre‑qualified via the PASSPort system and hold an HHS Accelerator PQL status. • Deliver primary medical, dental, and mental‑health services to K‑12 students on school premises. • Provide preventive care, chronic condition monitoring, and urgent care to reduce absenteeism and ER visits. • Operate under city‑approved protocols, reporting, and compliance requirements, with contracts awarded on a rolling basis.
