OPEN-ENDED - SCHOOL BASED HEALTH CENTER SERVICES (SBHC) RFP

The New York City Department of Health and Mental Hygiene is seeking qualified providers to deliver comprehensive primary and mental health services through School‑Based Health Centers (SBHC) on an open‑ended basis. Providers will staff, operate, and manage health centers located within NYC public schools, offering preventive care, chronic disease management, urgent care, and mental health counseling to K‑12 students in high‑risk neighborhoods. Proposals are accepted continuously via the PASSPort system, and only vendors with an approved HHS Accelerator Pre‑Qualification List may submit. The RFP emphasizes improving student health outcomes, reducing absenteeism, and decreasing emergency‑room utilization. • Deliver primary medical, dental, and mental‑health services on‑site at participating NYC schools. • Provide 24/7 on‑call coverage, chronic disease management, immunizations, and health education. • Staff each center with qualified clinicians (physicians, nurse practitioners, mental‑health counselors) and support personnel. • Meet performance metrics related to student attendance, health outcomes, and cost‑effectiveness. • Maintain compliance with NYS SBHC licensing, reporting, and data‑privacy requirements.
