OPEN-ENDED - SCHOOL BASED HEALTH CENTER SERVICES (SBHC) RFP

The New York City Department of Health and Mental Hygiene (DOHMH) is seeking qualified providers to deliver comprehensive School‑Based Health Center (SBHC) services on an open‑ended basis. Proposals are submitted through the city’s PASSPort system and must come from organizations that hold an approved HHS Accelerator pre‑qualification. The contract will support primary medical care, mental health counseling, preventive health services, and chronic disease management for students in high‑risk NYC schools, aiming to reduce absenteeism and emergency‑room visits. • Provide on‑site primary and preventive health care, including physical exams, immunizations, and chronic‑illness management. • Deliver mental‑health counseling, behavioral health services, and crisis intervention within the school environment. • Operate under NYS Chapter 198 requirements, maintaining licensing, reporting, and quality‑assurance standards for each SBHC. • Serve an indefinite number of schools until city needs are satisfied; proposals are reviewed continuously. • Must be pre‑qualified in PASSPort and maintain an active HHS Accelerator status throughout the contract term.
