OPEN-ENDED REQUEST FOR PROPOSAL - SCHOOL BASED HEALTH CENTER SERVICES

The New York City Department of Health and Mental Hygiene is seeking qualified providers to deliver comprehensive, preventive, and primary health services to students through School‑Based Health Centers (SBHCs) on an open‑ended, continuous‑submission basis. Providers must have a PASSPort account with an approved HHS Accelerator PQL status, and proposals are accepted exclusively through the PASSPort system. The contract supports medical, mental health, and preventive care for K‑12 students in high‑risk areas, aiming to reduce absenteeism, ER visits, and parental work loss. • Deliver on‑site primary medical and mental‑health services to students in NYC public schools. • Provide preventive care, chronic‑illness management, and urgent care during school hours. • Maintain compliance with NYS Department of Health licensing, reporting, and quality‑monitoring requirements. • Operate under a competitive sealed‑proposal, pre‑qualified‑list selection method with ongoing submissions. • Must have PASSPort HHS Accelerator pre‑qualification before submitting a proposal.
