AMENDMENT 0004- Onsite Phlebotomy Services- Houston, TX

The Department of Veterans Affairs is seeking a contractor to provide onsite phlebotomy services at the MEDVAMC in Houston, TX. The contract will be for a period of 12 months, with the option to extend. The contractor will be required to provide phlebotomy staffing services, including base hourly rate, holiday rate, and overtime rate. The estimated quantity of hours for services rendered is 33,280 hours. • The contractor will be required to invoice and payment is limited to the quantity of hours for services rendered. • The contractor will be required to provide phlebotomy services onsite at MEDVAMC. • The contractor will be required to reimburse the VAMC for the cost of background screens/investigations of each of the submitted candidates, but this requirement has been removed from the PWS.
