The Department of Veterans Affairs is seeking a vendor to provide a Flo-Lab Vascular System to the Lebanon VA Medical Center. The system must be a Flo-Lab Model 2100 SX-2 Vascular System. This is a total Small Business set aside. The vendor must provide a quote that includes a technical section, price section, and past performance references. The technical section must demonstrate the vendor's ability to meet the requirements of the Statement of Work (SOW). The price section must include a completed schedule and a detailed price breakdown. The past performance references must be for similar services provided within the past five years. • The quote must be submitted electronically via email to [email protected]. • The quote must include all required sections, including the technical section, price section, and past performance references. • The vendor must be registered in the System for Award Management (SAM) database at the time of quote submission and at the time of award. • The vendor must provide a certification regarding trafficking in persons compliance plan.
