Description
The State of North Dakota’s Department of Health and Human Services is seeking a consultant to deliver a comprehensive suite of services for the “ND Moves Together” initiative under its Rural Health Transformation Program. The consultant will manage participant onboarding, develop and train cohort groups, conduct monthly cohort meetings, document technical‑assistance site visits, collect and report participant‑level data, prepare outcome presentations, coordinate storytelling visits for awardees, and provide guidance for the “Fit, Focused, Friendly” media campaign. The base contract runs from September 2026 through August 2027 with three optional one‑year extensions, and a total potential value of up to $298,320.
