Description
The Department of Veterans Affairs, VA Hudson Valley Healthcare System, Castle Point VA Medical Center, intends to award a sole source contract to Cadwell Laboratories, Inc. for their Sierra EMG Machine. The contract will include the delivery of one EMG machine with various components and accessories. The period of performance is 30 calendar days from the date of receipt. The contractor will be responsible for furnishing and delivering the EMG machine, as well as providing on-site installation and training.
• The EMG machine will include a Sierra Summit Base with 1-2 Channel Amplifier
• A Sierra Summit 2 Channel License
• A Stim Troller Plus Stimulator
• Two Stimulator Probes
• A High Performance Laptop PC
• A Fuzion Cart for Laptop
• A Summit 1-2 Ch Amp Arm w/Stim, Amp & Needle holder
• A Fuzion Cart Footswitch Holder
• A Medical Grade 6 outlet Power Strip for Fuzion Cart
• A USB Footswitch
• On Site Installation with Training for 1 day
