Respite and Extended Services

The Connecticut Department of Public Health is seeking proposals to develop, administer, and serve as the fiduciary of funds for a statewide program to provide information about, and assistance with accessing and funding, services for children and youth with special health care needs. The program aims to supplement existing services provided by the Department’s CYSHCN program and the CT Medical Home Initiative. The scope of work includes assisting caregivers in accessing and funding respite services and extended services, enrolling CYSHCN in the CHMI Program, and obtaining health insurance for CYSHCN. A total of up to $2,057,765 is available to support one contract, with funding for three years and two one-year extensions, beginning July 1, 2026, through June 30, 2031.
