Community Pathways Services

The County of San Mateo's Human Services Agency is seeking proposals from qualified proposers to operate a Community Pathways Program. The program aims to provide prevention and early intervention services to families, with the goal of reducing child abuse and neglect. The selected contractor will be responsible for service coordination and case management, Family First Prevention Services (FFPSA) service delivery and oversight, and community saturation and co-location. The contractor will work with community-based organizations and County departments to address assessed needs and provide culturally responsive, trauma-informed, and family-centered services. Some key features of the program include: • Receiving referrals from multiple sources and conducting needs assessments • Providing service coordination, case management, referrals, and prevention services • Delivering evidence-based programs (EBPs) and coordinating referrals to partner agencies • Establishing partnerships with local schools and community organizations • Ensuring services are culturally and linguistically responsive, trauma-informed, and family-centered The County anticipates awarding a five-year agreement, with a tentative start date of July 2026. Key considerations for the proposer include: • Demonstrated experience in delivering prevention and early intervention services • Ability to collaborate with community-based organizations and County departments • Capacity to provide culturally responsive and trauma-informed services • Strong project management and leadership capabilities • Ability to meet performance measures and reporting requirements
