Description
The Alaska Department of Health, Division of Public Health, is seeking professional community outreach experts to support its efforts in public education, resource linkage, and patient navigation in Mat-Su, Alaska. The selected contractor will provide services to an average of 630 women per year who require patient navigation. The scope of work includes public education, resource linkage, and patient navigation. Key responsibilities may include:
• Developing and implementing outreach strategies to reach the target population
• Providing patient navigation services to clients
• Linking clients to resources and services
• Collaborating with healthcare providers and other stakeholders
• Evaluating the effectiveness of outreach efforts
