Description
The Ohio Department of Health seeks a vendor to provide oversight and implementation support for the Healthy Heart Ambassador Blood Pressure Self‑Monitoring (HHA‑BPSM) program within Pathways Community HUBs. The contractor will coordinate with Community Health Workers, Care Coordination Agencies, and local HUBs to improve blood‑pressure management among residents. This RFP requires both technical and cost proposals and emphasizes program management, training, data collection, and quality‑assurance activities.
• Provide program oversight, training, and technical assistance to CHWs and HUB staff.
• Coordinate with Care Coordination Agencies to ensure seamless referrals and follow‑up.
• Collect, analyze, and report blood‑pressure data to evaluate program outcomes.
• Develop and deliver educational materials and outreach strategies for community members.
• Manage contractual compliance, performance metrics, and fiscal reporting for the FY27 award period.
