Onsite and Offsite Physical Therapy and Employee Wellness Services

The City of Davenport is seeking a qualified vendor to provide onsite physical therapy services at the City's health clinic, as well as offsite physical therapy and employee wellness services. The selected vendor will deliver comprehensive physical therapy and wellness services to support employee well-being, workplace safety, regulatory compliance, and productivity. The scope of services includes clinical care, prevention, functional screens, and ergonomics, as well as wellness programs and initiatives. The vendor will be expected to provide a pricing proposal, outline their staffing model and cost structure, and demonstrate their experience and qualifications in delivering similar services. • The initial contract term will be two years, with the option of three additional one-year renewals. • The City employs approximately 800 full-time employees, 300 part-time employees, and 250 seasonal employees across 14 departments. • The selected vendor will be expected to provide services at the City's onsite occupational health clinic, the Total Wellness Center, as well as at least one offsite location within Davenport city limits. • The vendor will be required to provide education, ergonomic assessments, and proactive solutions to minimize future injuries. • The City will provide a ~3,500 sq. ft. clinic, the Total Wellness Center, located at 1200 E. 46th St. • The vendor will be expected to maintain complete and accurate records relating to the services and retain such records for the period required by the resulting agreement and applicable law.
