The City of Miami is seeking a proposal for an Employee Health Benefits Plan. The proposal should include information about the proposer's organization, experience, qualifications, capabilities, and financial stability. The proposer should also provide a detailed description of their proposed network and plan design, customer service, banking, reporting capabilities, and benefit administration. The evaluation criteria will include technical criteria, price, and cost-effectiveness. The contract term will be for 5 years with optional 3-year renewal periods.
